A preview row is a description, a count and a unit price in dollars. Bill labor by the hour at your hourly rate, materials as the number of units times the price of one, and a permit or disposal fee as a quantity of 1. If the invoice covers one stage of a larger job, say which stage in the first description.
There is one tax percentage in the preview and it covers the whole subtotal, so materials and labor can't be taxed at different rates there. Look up your state's rules, and get an accountant's view on presenting them. Deposits, change orders and Form W-9 are covered in the contractor invoicing guide linked below.
Worked example with the sample rows: 16 hours of demolition and haul-away labor at 16 × $60.00 = $960.00, 24 drywall sheets at 24 × $15.00 = $360.00, and a permit fee at 1 × $150.00. The rows add to $960.00 + $360.00 + $150.00 = $1,470.00, which is the total with the tax box left at 0. Use your own rates and prices.
