Put the retainer on its own row with the month in the description, extra hours on another row, counted in hours, and each purchase made for the client, such as a stock image license, on a row of its own at what you paid.

Worked example with the opening sample rows: a monthly retainer at 1 × $2,400.00, six extra design hours at 6 × $95.00 = $570.00, and two stock image licenses at 2 × $29.00 = $58.00.
