Service receipt

A receipt for a service visit: the service date, equipment and technician, the services performed and parts used in one table with quantity and rate, a sales tax rate cell, the total, the amount paid and the payment method.

Confirms a payment received

What's on the service receipt

The service receipt confirms that a customer has paid for a service visit. It has:

  • Service business: your Business name, Address and Phone / email.
  • Receipt details: Receipt no. and Receipt date.
  • Customer: Customer name and Address.
  • Service visit: Service date, Equipment and Technician.
  • Services and parts: one table with Description, Service or part, Qty, Rate and Amount.
  • Totals: Subtotal, Sales tax, if applicable, Total and Amount paid.
  • Payment: Payment method, Payment reference and Date paid.
  • The footnote "Thank you. Keep this receipt with your appliance records."

The receipt is marked PAID. It comes as three files:

  • PDF: a blank form to print and fill in by hand. It's sized for A4 paper, so if you print on US Letter, check the scaling setting in your print dialog so the whole page fits.
  • Word (.docx) and Excel (.xlsx): the same layout filled with sample details. Overwrite them with your own.

In the Excel file:

  • each row's Amount is Qty × Rate, rounded to the cent;
  • Subtotal adds up the Amount column;
  • a "Tax rate (enter your rate)" cell sets Sales tax, if applicable, which is the Subtotal × that rate (0% in the sample);
  • Total is the Subtotal plus the sales tax;
  • you type the amount received in Amount paid.

The file's tip says that for a service billed by the hour you enter the hours in Qty, to leave the tax rate at 0% if no tax applies, and not to delete the rows, because the totals refer to them. The table has four sample rows and four blank ones.

The sample works out like this, for a dishwasher repair on Nov 17, 2026:

  • Services: a service call and diagnosis at 85.00, and 1.5 hours to replace the drain pump at 90.00 an hour, 135.00;
  • Parts: the drain pump, 64.00, and a door gasket, 38.50;
  • a Subtotal of 322.50, with sales tax at 0%, and a Total of 322.50, paid by card the same day.

The sample prices only show how the form works, so use your own.

Services and parts in one table

Every line goes in the same table, and the Service or part column says which it is, so the customer can see what was labor and what was parts without a second table. Bill a flat charge, such as a service call, with Qty 1 and its price as the Rate. Bill hourly work with the hours in Qty, as the sample does with "Replace drain pump, hours" (1.5 at 90.00).

Fill in the Service visit block, so the receipt shows when the work was done, on what, and by whom. The sample's Equipment line reads "Dishwasher, [make and model]".

Sales tax on services and parts

The Sales tax, if applicable line applies one rate to the whole Subtotal, services and parts together. Whether labor, parts or both are taxable depends on the rules where you work, so check with your tax agency before you set a rate. If only parts are taxable for you, change the tax formula so it adds up the part rows only.

Payment details

Record how the customer paid in Payment method (the sample reads "Card ending [last 4 digits]"), the transaction or check number in Payment reference, and the Date paid. Amount paid is a number you type, so it shows what you actually received.

This receipt is for a visit paid in full. If the customer pays only part of an invoice, the invoice payment receipt is set up for that, with the amount paid now, the total paid to date and the balance remaining.

Service receipt or a trade receipt?

  • This service receipt: a general service visit, with services and parts in one table at a quantity and rate, the equipment and technician, and one sales tax rate on the whole subtotal.
  • Mechanic receipt: a vehicle repair, with the year, make and model, VIN, license plate and odometer, a disposal fee line and a balance after the payment.
  • Handyman receipt: a handyman job, with each piece of work and its location, a deposit paid earlier and a balance after the payment.
  • Plumbing receipt: payment of a plumbing invoice, with the invoice number, date and total, a deposit paid earlier and the amount paid now.

Other receipt templates include the payment receipt, the online order receipt and the generic receipt.
