Physical Therapist and Physiotherapy Invoice Template

A rehabilitation program might start with an assessment and continue with shorter follow-up sessions, sometimes paid one at a time and sometimes as a prepaid package. Clients may pay part of the bill at the front desk on the day, and some invoices go to a parent or another person who is responsible for payment rather than to the client. The sections below show how to put each of these on an invoice, using the physiotherapy session invoice template and synthetic figures. The physiotherapy session invoice

For practices that charge by session length at an hourly rate, Billed's physiotherapy invoice template is the file set this page uses. It names the practice and the treating therapist, gives the client an account number, and has a bill-to block for the client or the responsible party, with the payment type shown as private pay. Every session is a separate row showing the date, the type and length of the session, the hours and the hourly rate. Session packages have a separate table, and money already received comes off before the amount due. Its footnote reads: "This invoice shows charges only. It is not an insurance claim form and contains no clinical information."

Here is how its sample adds up. Four visits between November 3 and 12, 2026 are billed at the hourly rate by length: a 60-minute initial assessment at 130.00 an hour (130.00), then follow-ups of 45, 45 and 30 minutes at 120.00 an hour (90.00, 90.00 and 60.00), which makes 370.00 for the sessions. A package of six 45-minute follow-ups for use from November 17 costs 480.00, so the invoice totals 850.00. The client paid 130.00 at the first visit, leaving 720.00 due. Spread over six sessions, the package works out at 80.00 each, against 90.00 for a single 45-minute follow-up in the same sample.

Dates of service, session length and units

Give every visit its own line dated with the day the session took place, even when several visits go on one invoice. The physiotherapy session invoice charges by length at an hourly rate, so a 45-minute session is 0.75 hours and a 30-minute session is 0.5 hours.

Some practices price by time unit or by visit instead. In that case each line needs the number of units and the rate per unit: for example, 3 units at 40.00 come to 120.00. The physiotherapy session invoice has no units column, because it charges by hours.

Copays, deposits and self-pay balances

When a client pays something on the day, such as a copay or a deposit, record it as a payment received so it comes off before the amount due. For example, a self-pay client with three 45-minute follow-ups at 90.00 each owes 270.00, and after paying 40.00 at each visit (120.00 in all), 150.00 is left to pay.

If an insurer is billed for the visits, the client's invoice should show only what the client is being asked to pay, and payments still expected from an insurer shouldn't appear as if they had been made. Home visits, packages and other related templates
