How do I bill work that isn't on my flat-rate list?

Add it under additional labor at your hourly rate, with what the work was. The sample bills 1 hour at 90.00 to realign a vertical track. Agree work like that with the customer before you do it.

How do I show a warranty on the invoice?

Write your terms in the repair notes: what's covered and for how long. The sample's terms, 1 year on springs and opener parts and 90 days on labor, are sample terms only. The rules on warranties depend on where you work, so check them.

Is sales tax charged on garage door repairs?

It depends on where you work. The template's "Sales tax, if applicable" rate starts at 0% and applies to the whole subtotal in the Excel file. If only parts are taxable where you work, check with the tax agency there.

Garage door invoice or home service invoice?

Both can bill a garage door job, but they price it differently.

This garage door repair invoice prices each repair as one flat-rate line with parts and labor included, such as "Extension spring replacement, pair" at 245.00, and adds hourly labor only for work outside those prices. It has a "Door and opener" block for the door, spring system, opener make and model, and opener serial number. Sales tax, if it applies, is on the whole subtotal.
The home service invoice is for any trade. It bills a call-out fee, then labor by the hour and parts with their own prices and subtotal, with sales tax on parts only. Its sample is a torsion-spring replacement billed that way.

That's a subtotal of 674.00. With sales tax at 0%, 674.00 is due on completion. The prices are sample figures, not market rates.

Flat-rate repairs and additional labor

A flat rate tells the customer the price of a repair before you start, whatever it takes to fit. The sample's first note says what the price covers: "Flat-rate repairs include the parts and the labor to install them." Put what the repair involved in "Details", such as "With new safety cables" or "Stripped drive gear replaced", so the customer can see what they paid for.

Use "Additional labor" for work your flat rates don't cover, like the sample's track realignment, billed by the hour. Agree any work like that with the customer before you do it.

Your flat-rate prices, and what each includes, are your business terms. Keep a price list, so you charge the same repair the same way each time.

Door and opener details

The "Door and opener" block records what you worked on: the door's type and size, the spring system, and the opener's make, model and serial number, which are placeholders to copy from the opener. With "Problem reported", it gives the customer a record of the visit, and gives you one if they call you back about the same door.

Warranty terms

The sample's warranty is marked as sample terms: "Sample terms: springs and opener parts installed on this visit are covered for 1 year, and labor for 90 days, from the service date." Write your own warranty on the invoice, saying what's covered and for how long, and keep it the same as what you told the customer. If parts also carry a manufacturer's warranty, say so separately. The rules on warranties depend on where you work, so check them before you set your terms.

The last note says what happened to the parts you took out: "[removed and recycled, or left with the customer]". Pick the one that applies.

License number and sales tax

"License no." is a placeholder for your license number, if the rules where you work require you to show one. Check them.

"Sales tax, if applicable" starts at 0%, and in the Excel file it applies to the whole subtotal. Whether repairs, parts or labor are taxable depends on where you work. If only parts are taxable there, a flat-rate price that includes labor may need splitting, so check with the tax agency there, then set the rate or change the formula.

How to fill in the template

Add your business details and license number if required, then the invoice number, invoice date, payment terms, due date and work order number.
Enter the customer and service address under "Bill to (customer)".
Fill in the "Door and opener" block and the problem the customer reported.
Add the service call, each flat-rate repair with what it involved, and any additional labor hours.
Set the sales tax rate, or leave it at 0%.
Edit the repair notes with your own warranty terms and what happened to the old parts, replace the placeholders in "How to pay", and give the customer the invoice.

The PDF is a one-page blank form for filling in by hand. It's sized for A4 paper, so if you print on US Letter, check the scaling setting in your print dialog so the whole page fits. The Word and Excel files have the same layout with the sample filled in, for you to overwrite.

In the Excel file:

each line amount is "Qty" × "Rate", rounded to the cent;
the three section subtotals, "Subtotal", the sales tax and "Amount due" are formulas; the sales tax is its rate × the subtotal;
to leave out sales tax, set its rate to 0%. Don't delete the row, because the amount due refers to it;
the service call table has no blank row, and the flat-rate repairs and additional labor tables have one each. For more lines, insert a row inside the table and copy the amount formula into it, then check that the subtotals include it.

In Word or on the PDF, work out the amounts yourself.
