PDF service invoice

A print-and-fill service invoice with room to write in each visit by date, the service, hours and hourly rate, a tax rate cell and the amount due.

A request for payment — shows the amount due

In the Excel file, each line amount is a formula: "Hours" × "Rate". The subtotal, tax and amount due are formulas too. The tax rate sits in its own cell, so you can change it.

PDF product invoice

A print-and-fill product invoice with the quantity first on each line, the item code, description and unit price, a ship-to address, a delivery fee, a tax rate cell and the amount due.

A request for payment — shows the amount due

In the Excel file, each line amount is a formula: "Qty" × "Unit price". The subtotal, tax and amount due are formulas too. The tax rate sits in its own cell, so you can change it.

Two blank PDF invoices

This page has two PDF invoice forms:

  • PDF service invoice: for work billed by visit, with Date, Service, Hours, Rate and Amount columns.
  • PDF product invoice: for goods, with Qty, Item, Description, Unit price and Amount columns, plus a Ship to block and a Customer PO no.

Each PDF is a single A4 page with 12 empty lines for services or items. The PDFs have no fillable fields: they're blank forms to print and fill in by hand.

Each invoice also comes as a Word (.docx) and an Excel (.xlsx) file with the same layout, filled with sample details. Use those files if you'd rather type your invoice.

The PDF service invoice

The service invoice has:

  • From: Business name, Address and Phone / email.
  • Invoice details: Invoice no., Invoice date, Payment terms and Due date.
  • Bill to: Name, Address and Phone / email.
  • Services: one line for each visit or job, with Date, Service, Hours, Rate and Amount.
  • Totals: Subtotal, Sales tax, if applicable with a blank for the rate, and Amount due.
  • How to pay: Accepted methods, Make checks payable to, Bank details and Payment reference.

The Word and Excel samples bill an office for four cleaning visits in November at 45.00 an hour:

  • 3 hours on Nov 3, Nov 10 and Nov 24 (135.00 each) and 4 hours on Nov 17 (180.00);
  • a Subtotal of 585.00, sales tax at 0%, and an Amount due of 585.00 by Dec 15, 2026 on Net 15 terms.

In the Excel file, each Amount is Hours × Rate. The Subtotal, the sales tax (the Subtotal × the rate you enter) and the Amount due are formulas.

The PDF product invoice

The product invoice has the same From, Invoice details and How to pay blocks, plus:

  • Customer PO no. in the invoice details, for buyers who issue purchase orders.
  • Bill to: Name, Attention and Address.
  • Ship to: Name, Address and Delivery date, for when the goods go somewhere other than the billing address.
  • Items: Qty first, then Item (your item code), Description, Unit price and Amount.
  • Totals: Subtotal, Delivery, Sales tax, if applicable with a blank for the rate, and Amount due.

The samples bill a school for art supplies:

  • 24 sketchbooks at 6.50 (156.00), 12 watercolor sets at 14.75 (177.00), 30 brush sets at 4.20 (126.00) and 6 easels at 22.00 (132.00);
  • a Subtotal of 591.00, plus 18.00 delivery, sales tax at 0%, and an Amount due of 609.00 by Dec 12, 2026 on Net 30 terms.

The payment reference asks the buyer to quote both the invoice number and the PO number. In the Excel file, each Amount is Qty × Unit price, you type the delivery charge, and the sales tax applies to the Subtotal plus Delivery at the rate you enter.

Whether sales tax applies, and to what, depends on your state and local rules, so check with your tax agency. The sample prices only show how the forms work.

How to fill in and send the PDF

  1. Print the page. It's sized for A4 paper, so if you print on US Letter, check the scaling setting in your print dialog so the whole page fits.
  2. Fill in your details, the invoice number, invoice date, payment terms and due date, and the client's details.
  3. Write one line for each visit or item, and work out each Amount: Hours × Rate on the service invoice, or Qty × Unit price on the product invoice.
  4. Add the lines for the Subtotal. On the product invoice, add any delivery charge. If sales tax applies, write the rate in the blank, work out the tax, and write the Amount due.
  5. Fill in How to pay, keep a copy, and hand the invoice over, mail it, or scan it to send by email.

Give every invoice its own number, in sequence, so you and the client can refer to it.

The PDF doesn't add anything up for you. Check the arithmetic before you send it, or use the Excel file, which calculates the amounts and totals.

Typing your invoice and saving it as a PDF

To send a typed PDF, fill in the Word or Excel version and save it as a PDF. Microsoft's instructions for Office desktop apps say:

  • Word: select File > Save As (or Save a Copy), choose PDF in the drop-down list, then Save.
  • Excel: select the File tab, then Save As or Save a copy, and choose PDF in the Save as type list.

The Word file holds typed values only, so update the amounts and totals yourself. The Excel file recalculates them.

You can also create and send invoices from Billed, which publishes this page. Billed connects to Stripe and PayPal so clients can pay online, and only their standard rates apply (accept payments).
